Physiotherapy jobs in Germany for Indian physiotherapists
An Indian physiotherapist can work in Germany, but "Physiotherapeut" is a regulated profession: you need a German licence (Berufsurkunde) issued by the health authority of a federal state before you may practise under that title. In practice the route is — learn German to B1 so you can interview and apply for a visa, get an employer and a recognition decision (Anerkennungsbescheid), move to Germany on a recognition or skilled-worker visa, reach B2 and complete the adaptation measure your decision names (usually an Anpassungslehrgang or a Kenntnisprüfung), then receive full licensure. Most candidates who start from zero German need 12–24 months.
Is physiotherapy a regulated profession in Germany?
Yes. Physiotherapy is one of the regulated health professions (Gesundheitsfachberufe). Without state recognition you may not call yourself Physiotherapeut/in or bill treatments. Your Indian BPT or MPT is not automatically equivalent — the authority compares your degree against the German three-year Ausbildung and issues either full recognition or, far more often, a partial recognition listing the gaps you must close.
The two ways to close the gap
| Anpassungslehrgang | Kenntnisprüfung | |
|---|---|---|
| What it is | Supervised adaptation period at a German clinic or practice, with accompanying theory | A single state examination covering the German curriculum, written/oral and practical |
| Typical length | Commonly 6–12 months depending on the gaps identified | Preparation course of a few months, then one exam date |
| Paid? | Yes — you work as an assistant while adapting | Normally not; you study for the exam |
| Best for | Candidates who want income and a guaranteed employer from day one | Candidates who want the fastest possible route to full licensure |
Your recognition decision tells you which options you may choose from. The choice is yours in most states, and most Indian therapists placed through MedUniverse take the Anpassungslehrgang because it comes with a salary and a contract.
What German level do you need, and when?
- B1 — enough to hold employer interviews and to support a visa application. This is the level to target first; it is what unlocks the process.
- B2 — the level the state health authorities normally require before granting the licence, and what employers expect on the ward. Plan for B2 before your adaptation period.
- C1 — needed if you enter through a German Ausbildung (vocational training) route rather than recognition of your existing degree.
Requirements are set per federal state. Always confirm the level your target state demands before booking an exam — see Which German level do you actually need.
Step by step: from India to a German clinic
- Start German — Begin A1 and work through to B1 with a medical-German focus so the vocabulary you learn is the vocabulary you will use.
- Prepare documents — Degree certificate, transcripts, syllabus/curriculum, registration certificate, experience letters and passport — translated by a sworn translator and apostilled.
- Choose a Bundesland and apply for recognition — Recognition is granted per state. File with the competent authority of the state where you intend to work.
- Interview with employers — German clinics and practices interview at B1 and issue a contract or letter of intent conditional on recognition.
- Apply for the visa — Recognition visa (§16d) if you still have an adaptation measure to complete, or a skilled-worker visa (§18a) once fully recognised.
- Relocate and complete the measure — Reach B2, complete the Anpassungslehrgang or pass the Kenntnisprüfung, and receive your Berufsurkunde.
- Work as a fully licensed Physiotherapeut — Move onto the full pay scale and, after the qualifying period, apply for permanent residence.
What do physiotherapists earn in Germany?
Pay depends on the collective agreement, the state and your experience. Public hospitals pay under TVöD; private practices vary. Entry-level physiotherapists typically start in the region of €2,700–€3,400 gross per month, with higher figures in hospital settings and after specialisation. During an Anpassungslehrgang you earn less, because you work as an assistant until licensure. Check current figures in the Entgeltatlas and the TVöD tables, and estimate your take-home with our salary calculator.
What does it cost?
The main costs are language training, sworn translation and apostille of your documents, the recognition fee charged by the state authority, the visa fee, proof of funds (a blocked account if you enter on a recognition visa), flights and your first weeks in Germany. See the full breakdown in Total cost of relocating from India.
How long does it take?
From your first German class to your first day at work is commonly 12–24 months. Language is the longest and most controllable part; recognition processing and visa appointments are the least predictable. The detailed month-by-month view is in Timeline: first German class to first day at work.
Frequently asked questions
Can I work in Germany as a physiotherapist without recognition?
Not under the protected title. You may enter on a recognition visa and work in an assistant capacity while completing your adaptation measure, but full practice requires the licence.
Is an Indian BPT accepted in Germany?
A BPT is accepted as the basis for a recognition application. Because the German qualification is a three-year vocational training with a different practical structure, most Indian degrees receive partial recognition with an adaptation measure attached.
Do I need a job offer before applying for recognition?
No — you can apply for recognition first. But having an employer helps: many clinics host the Anpassungslehrgang and support the visa.
Does an MPT shorten the process?
It can strengthen your file and help with specialised roles, but it does not remove the requirement for German licensure or the language level.
MedUniverse guides Indian physiotherapists and occupational therapists through this end to end — German language training up to B2, document translation and apostille, the recognition file, employer interviews and relocation. Book a free consultation or see current openings.
