Recognition authority by Bundesland — which state should you apply in?
Recognition for physiotherapy and occupational therapy is granted by the health authority of a single German federal state, and you may only have one application running at a time — but the licence you receive is valid across all of Germany. The right state is normally the one where your employer or adaptation clinic is located, not the one with the fastest reputation, because the adaptation measure and any exam are organised locally.
How to find your authority
Use the federal Anerkennungs-Finder: select your profession (Physiotherapeut/in or Ergotherapeut/in), your country of qualification and the state you plan to work in. It returns the competent authority with its address, forms and fee schedule. Depending on the state this is a Landesprüfungsamt, a Regierungspräsidium, a Landesamt für Soziales or a Bezirksregierung.
What actually differs between states
- Processing time and how strictly the file is reviewed
- The fee charged for the assessment
- Which adaptation options they offer and how many Kenntnisprüfung dates exist per year
- Exact language evidence accepted for the licence
- How many clinics in that state routinely host Anpassungslehrgang places
One application at a time. Filing in two states does not speed anything up and can invalidate both files.
A practical way to choose
- Find the employer or adaptation clinic first — through our job board or your own search
- Apply for recognition in that employer's state
- Confirm with the authority which language certificate they accept before booking an exam
- Only consider moving states later if your employment changes
